Implant is a replacement for root or roots of a tooth. Just as the roots hold the tooth firmly into the jawbone, so do the implants. Fabricated in Titanium, a lightweight, biocompatible metal, implants securely hold the crown/bridge or dentures over them. This is attributed to ‘Osseointegration,’ a property of Titanium, which helps the implants to fuse with the surrounding bone, to make a stable unit.

The ABC of Implant Prosthesis-

The structure of the implant-supported prosthesis comprises of

A. Implant: This part goes inside the jawbone and fuses with it
B. Abutment or the connector
C. Custom made crown, bridges or implant supported dentures

At TnSS, we believe in the accuracy of treatment, and thus prefer to do guided implant surgeries by using Surgical guides, fabricated with the help of Implant Planning software, for utmost precision.

We, at our clinic, follow three types of techniques for dental implant surgery-

Single Day Implant- In this procedure, extraction of the natural tooth, placement of implant as well as the overlying prosthesis is given in the same go. It however depends on certain factors like the quality and quantity of the bone available.

Immediate Implants- This procedure involves placement of implants, immediately after extraction of the natural tooth, in the same sitting. The overlying prosthesis is given after a few days.

Two-Stage Implants- In this technique, in the 1st stage extraction is done and the site is left to heal for some time. Then implants are placed and left for 2-3 months. After required bone growth and healing is achieved, the abutment is screwed to the implant. The unit now becomes ready to hold the overlying prosthesis.

This technique, developed by Paulo Malo, involves placement of 4 implants in an edentulous or teeth less jaw, providing support for a fixed prosthesis.

At times, there might be a loss of bone due to loss of teeth or other anatomical limitation, which poses a problem with the placement of implants. The concept of all on 4 treatments provides a minimally invasive solution with a fixed full-arch restoration. In this method, 2 implants are placed in the anterior region and 2 in the posterior, which are tilted at an angle, to overcome bony deficiencies or anatomical structures, to achieve safer, simpler, graft less solution for completely missing teeth. The 4 implants act as pillars to the bridge that supports the prosthesis over it. The procedure takes advantage of the existing basal bone, even if scarce and makes it possible to have a fixedfull-arch prosthesis without involving bone grafts, sinus lift surgeries and nerve repositioning in the lower jaw.

A 3D CT scan is required prior to the procedure for secure and successful planning. The advantages of the procedure are-

Fixed Single/Multiple Teeth Replacement by Crown and Bridge

Proper restoration are the key to a confident smile. Restoration should be such that it enhances both the esthetics and function as well. At TnSS, we have a specialized staff dedicated to planning, designing and fabricating a full mouth reconstruction by crown and bridge over implants, perfect in both aesthetics and function.

A single tooth to full arch can be reconstructed over implants. The crown and bridge are made in PFM (porcelain fused to metal) and Zirconia, depending upon the patient’s preference, at our In-House dental lab, with perfect fit and precision. The prosthesis is fixed over implants by screws or cement, which then acts as a single functional unit.

Hybrid Dentures

The flexibility of implant therapy has increased over the years, presenting many treatment alternatives. One of them is Hybrid Denture. The screw-retained hybrid denture is a fixed full arch implant solution, that can only be removed by the dentist, and is thus essentially a permanent restoration simulating the look, feel and function of natural dentition.

Fabrication – At first, just like in conventional denture, a wax setup is made and tried for interocclusal records in the patient’s mouth. When the wax setup is verified, our In-House Dental lab uses the CAD CAM technology, to design and mill a Titanium framework. The framework could also be casted in Cobalt Chrome. This bar or framework is supported by the implants. The final wax setup is transferred onto the bar, to confirm the passive fit. After try in, the combination of wax setup and the metal bar is set in the acrylic base and denture teeth are added, producing a hybrid denture with precise fit and comfort.

Implant Supported Removable Denture

In the process of rehabilitation of edentulous patients, depending on their preference and condition, treatment modalities like fixed or removable dentures over implants are available nowadays. Implant supported removable denture offer a multitude of benefits of implant based therapy- biological, biomechanical, psychological and social. Moreover, implants supporting an overdenture provide retention, stability and an affordable alternative to conventional dentures. Apart from transition towards better speech and function, implant overdentures also increase confidence, comfort and emotional well being.

Fabrication- These dentures are removable appliances, attached to the implants by retentive caps or clips. By stabilizing the dentures, these devices improve function and chewing efficiency. After placement of implants, patient’s existing denture may be retained as a temporary measurement. Also, immediate denture can be given after extraction, as a provisional appliance. Once the osseointegration has taken place, restorative phase begins. We take impressions and send it to our In-house CAD CAM lab, for wax set up, to seat over locator abutments. When ready, setup is tried in the patient’s mouth. Then after fit and alignment are verified and jaw relationship recorded, final denture with retentive caps is fabricated, providing a snug fit over implants.

The dentures may be fabricated in two ways-

Bar Retained Denture
Bar Retained Denture – A thin metal bar is attached to the implants by clips or other types of attachments. The denture fits over the bar and is securely clipped into place by attachments.
Bar Retained Denture
Ball retained denture – In this, each implant holds a metal attachment, which fits into another attachment on the denture. The attachment on the implants is ball shaped and those in the denture have sockets, into which they fit.

Just like a regular denture, these should also be cleaned regularly, and taken off while sleeping.
